• Your order is not complete until we have received the following: your completed Medication Order Packet or online registration, your prescription insurance information, payment information and original prescriptions mailed to CampRx, 1179 Maplelawn Dr. Troy, MI 48084, if applicable. PARTIAL ORDERS WILL NOT BE PROCESSED.
• I understand that if I proceed with online registration, my camper’s order is not complete until either CampRx receives prescriptions from the doctor for a fee or the prescriptions are mailed to CampRx, 1179 Maplelawn Dr. Troy, MI 48084.
• You are responsible for reviewing all prescriptions and confirming that they are correct. If there is a discrepancy between what you have ordered and what the physician has written we will attempt to contact you for clarification, but in all cases we are required to fill the prescription exactly as the physician has ordered.
• Completed orders are due at CampRx 30 days before your child’s camp session begins. It is never too early to place your order. Late orders are subject to an additional processing charge of between $10 and $20. PLEASE NOTE: Late orders may also result in additional late shipping charges, which can be substantial because we ship all orders overnight via UPS.
• We will keep you informed of the status of your order via email. You will receive a confirmation email from us once we have received your Medication Order Packet or online registration. If you do not receive a confirmation email, that means we have not received your order and it is your responsibility to contact us immediately. We will also contact you when your order has been shipped to camp. If you do not receive a completed order email, it means your order is not being processed. If you do not receive a shipping notice before your child leaves for camp it means that no medications have been shipped. In both cases it is your responsibility to contact us to check on the status of your order.
• If you or your child’s physician do not indicate that brand name medication must be provided, our pharmacy will substitute a generic equivalent. If you or your child’s physicians do not indicate a dosing time, medication will be dosed in the morning.
• We do our best to supply the specific vitamins and supplements you order if you indicate that the brand name is required. If we cannot do so, or if the expense of providing it is prohibitive, we will advise you and it is your responsibility to contact your child’s camp to determine how to proceed.
• We can prepackage OTC medications you supply to us if they are unused in sealed containers, and if we receive them by the due date for completed orders. We will add a $5 prepackaging fee for each medication.
• We cannot fill or ship your order without current prescription insurance information unless you instruct us that you will pay cash for the medications.
• You are responsible to pay our pharmacy whatever amount is not reimbursed by your insurer, whatever the reason. If your insurer will not provide any coverage for prescription medications ordered through our pharmacy (for example, some insurers have a closed pharmacy network), we will let you know and contact camp to recommend that you be permitted to opt out of the program.
• Your credit card will be charged for the cost of your medication copays, any OTC medications, CampRx service fee and late/shipping fees if applicable. Please note that we cannot use HSA credit cards to bill you for OTC items or the CampRx fee. If you would like us to use your HSA card for your prescription medications, please provide us with another card for the other charges.
• We will email you an e-invoice with a detailed breakdown of your medication charges after the billing is completed. If you need additional or different documentation for any reason we will be pleased to supply it.
• We may be required to share medical information regarding your child with your child's physician, your home pharmacy, and your child's camp for the purpose of filling your order efficiently and accurately. Under applicable law, we are permitted to use and disclose health information for reasons such as treatment, payment or healthcare operation purposes. You agree and consent to DirectRx (CampRx’s pharmacy) using or disclosing medical information about your child for these purposes.
